HOW TO GET A MEDICAL MARIJUANAS CARD IN IL?

And lastly, get a letter from your doctor stating that you have one of the qualifying conditions and are considering using medical marijuana to aid in your treatment. First, you need an application form for an illinois medical marijuana card. You can download the form from the state of Illinois Department of Public Health website. The form is called an “Application for Certification” and it’s labeled #102a under Illinois medical cannabis and may take 30 days with application fees. Once you print out the form, fill it out and make sure to sign it. It also has to be notarized. Then, Illinois residents have to take your form to your doctor. The doctor needs to fill out Part B on the form, which is labeled “Physician Certification.” It should be filled out in blue ink and signed by your doctor. Your doctor may ask you some questions about why you want a medical cannabis cards and not recreational cannabis, so make sure you are prepared with an answer! Once your doctor has filled out Part B on the application, they will give it back to you. Then patient in Illinois will take their application back to the Department of Public Health, along with a copy of all of your medical records – this includes any previous applications that you’ve already made.
